Authorization for Consumer Credit Report Form
This form is a release form, to be completed by the job applicant, which allows your business to have a background check performed. The form must be completed and returned to Info Quest with an order form before the any screening services can be performed. The form also provides Info Quest with the necessary information to perform screening services.   [PDF Form]
