This the standard enrollment
form to recieve Info Quest's services. The form provides Info Quest
with information about your business and is the service agreement

This form is a release
form, to be completed by the job applicant, which allows your business
to have a background check performed. The form must be completed
and returned to Info Quest with an order form before the any screening
services can be performed. The form also provides Info Quest with
the necessary information to perform screening services.

This form is to be completed
by your business. It provides all the screening services available
and lists the prices for each service. The form must be completed
and returned with the job applicant's authorization form before
screening services can be performed. This form must be signed and
dated by an authorized company official to be vaild.

Card Authorization Forms
For individuals, we provide
a single time authorization form. This allows InfoQuest to charge
your credit card for a single transaction. [PDF
